APPLICATION & SUBMISSION CRITERIA

Expanding Your Practice to Include CHIP: An Interactive Workshop can only accommodate a limited number of physicians. For an application to be considered, the applicant must be a currently licensed US or Canadian interventional cardiologist (now and at the time of the program). Paid attendance is not an available option.

Program Policies

There is no registration fee to attend the program; however, full participation in all aspects of the program are required. To those accepted physicians who attend the full program (check-in will be noted), CRF will reimburse post program up to $600 USD (including taxes and fees) for standard economy airfare. Flights must be booked and paid for by the attendee and follow the Cardiovascular Research Foundation (CRF) provided travel parameters and instructions or travel will not be reimbursable. Further details on booking travel will be provided in the acceptance letter.

Additionally, the nights of August 10th and 11th in a Standard King Room at the Grand Hyatt Seattle will be booked and covered by the organization for accepted attendees. Any additional hotel arrangements or special requests are the responsibility of the individual and cannot be guaranteed. Meals and breaks will be provided during the program. Ground transportation, meals outside of program functions, the guest’s travel/expenses, and other miscellaneous expenses are not reimbursable and are the responsibility of the attendee.

Reimbursement forms will be provided after the program. Full participation and attendance, completion of the provided program evaluation form, and a completed reimbursement form is required in order to be reimbursed.

Additional information will be provided upon acceptance. Any arrangements made outside of CRF’s policies or prior to acceptance will not be reimbursed or covered.

Sunshine Act Compliance

This activity is not CME-accredited, and therefore, the supporter is required under the Sunshine Act (as part of the Affordable Care Act) to track and report all payments and transfer of value to US licensed physicians including meals and expenses that are provided to you. In order to help supporters comply with the Sunshine Act, the names and contact information of all US and Canadian licensed physicians participating in this function will be provided to the supporter of this program. The information provided will be used for reporting purposes only.

Should you partake in this program, you will be reported as part of the Sunshine Act submission and will appear on the CMS Open Payments website on the next reporting cycle.

Vermont-Licensed HCPs
Vermont laws prohibit this program from providing any food, meals, or refreshments at no charge to healthcare professionals licensed by and regularly practicing in Vermont. Accordingly, healthcare professionals licensed by and practicing in Vermont are requested not to apply for this program.

EXPLANATION OF APPLICATION PROCESS

When will I receive notification?

Licensed US and Canadian interventional cardiologist will receive an email notification letting you know if you’ve been accepted into this program. Please read this letter carefully.

If you are accepted, you will need to respond for your place to be held. Accepted registrants are required to participate in the FULL program, including providing a case. Late arrival and/or early departure are not allowed. If for some reason you are unable to attend, please email chip@crf.org immediately, as your spot will be issued to another applicant.

Note: Please do not contact CRF to confirm the status of acceptance. All correspondence will come from Ryo Mochido to the email provided in the registration form. Please add chip@crf.org to your safe sender list to ensure you receive all communications.

My application has been accepted. What happens next?

If you are accepted, you will receive an acceptance confirmation letter via email from Ryo Mochido. This letter will include information about housing details and travel arrangements. Please read this letter carefully!

Please do not book any airfare or hotel rooms prior to receiving an acceptance email, as you will not be reimbursed unless you use our specific companies and follow the requirements provided.

My application has not been accepted. Are there other options available to attend this course?

At this time paid registration is not available for the program. This being said, in some cases spots may open up at a later date. If this is the case, notifications of acceptance may be sent afterwards.

What is the meeting cancellation policy?

If you have been accepted, we ask that you do not cancel. If for some reason you must cancel, please notify CRF immediately by emailing chip@crf.org.

Physicians who cancel their attendance to this program will not be reimbursed for any expenses including travel, and based on the date of cancellation, may be subject to hotel (or other) charges incurred by the program organizer.

Attendance Arrangements:

Travel Arrangements

Physicians attending the program must arrive at the venue on Thursday, August 10th by 11:00 AM (PT) and depart no earlier than 6:00 PM (PT) on Friday, August 11th.

Booking instructions will be sent in acceptance letters. Note that accepted participants are required to participate in the full program as a stipulation of their attendance.

Accepted attendees will be reimbursed the cost of the airfare (up to a maximum of US $600.00 for standard economy airfare only) after attending the full meeting, submitting a case, completing the evaluation, and submitting a reimbursement form.

Housing at the Grand Hyatt Seattle

Housing (room and tax only) for the nights of August 10th and 11th in a Standard King Room will be booked and paid for by the course sponsor for physicians who have been accepted and have confirmed their travel (per guidelines). In the event of a late cancellation, a no-show, or an early departure/late arrival, an invoice will be issued for any charges incurred by CRF.

Extra Person Charge: Each extra person above the maximum occupancy (listed below) will be charged an additional $25.00 plus tax per night for a roll-away bed (maximum one roll-away per room). There is no charge for children who share the room with their parents, as long as the maximum occupancy is not exceeded. Maximum occupancy per room is four (4) adults in a room.
